When do you need lip and tongue tie surgery?

Not every tongue tie or lip tie needs surgery. Many children have mild restrictions that don’t interfere with normal function.
A frenectomy (tongue or lip tie release) is considered only when the tie affects important daily functions such as:

  • Feeding difficulties – trouble breastfeeding, prolonged feeding times, clicking sounds while nursing, poor weight gain, or maternal nipple pain.

  • Speech concerns – unclear pronunciation, difficulty saying certain sounds, or restricted tongue movement noticed during speech development.

  • Dental or oral issues – spacing between front teeth, gum recession, or difficulty maintaining oral hygiene due to tight frenum attachment.

  • Swallowing or chewing problems – messy eating, preference for soft foods, or poor tongue coordination.

  • Airway and sleep issues – mouth breathing, snoring, or sleep-disordered breathing related to restricted tongue posture.
